The Permanent Housing Advanced Clinical Services (PHACS) program brings trauma-informed, wrap-around mental health and behavioral health support to permanent supportive housing sites across San Francisco. What You Bring
- Master's or Doctoral degree in Social Work, Psychology, Counseling, or a related field
- Current licensure as an LCSW, LMFT, LPCC, or Psychologist in good standing
- Eligibility to supervise clinicians (2+ years post-licensure and supervision credential, or willingness to obtain within 90 days)
- Minimum of 2 years' experience in direct clinical work and supervisory roles
- Strong knowledge of trauma, harm reduction, substance use, and systems of oppression
- Skilled communicator who thrives in diverse, collaborative, and high-impact environments
- CPR and First Aid certification (or ability to complete within 90 days)
- TB clearance and proof of COVID-19 vaccination within 7 days of hire

- Personally cultivate and steward a portfolio of 50–75 major gift prospects and donors capable of giving $25,000+ annually.
- Partner with the Co-CEOs, Board, and Campaign Manager to drive donor engagement for a comprehensive $15M+ campaign, including major gifts, legacy giving, and institutional partnerships.
- Oversee and innovate across all fundraising streams, including events, donor communications, corporate engagement, and new philanthropic partnerships.
- Lead development of fundraising dashboards, campaign reports, donor pipelines, and grant calendars.
Grow and Inspire Your Team
- Supervise, mentor, and grow a high-performing Development team.
- Foster a culture of collaboration, boldness, and equity within the department and across the organization.

- Supervise and coach Program Directors, building a supportive, trauma-informed culture
- Ensure staff training, development, and performance evaluation prioritize empowerment and healing
- Champion staff with lived experience and foster a culture of inclusion and professional growth
Coordinate Quality Care
- Maintain policies and protocols that ensure client-centered, culturally responsive, and trauma-informed services
- Collaborate across housing, healthcare, and social service partners for seamless client support
- Work closely with the Department of Public Health and others to meet integrated care goals
Lead Compliance & Reporting
- Oversee regulatory compliance and reporting to funders, ensuring program integrity
- Implement continuous quality improvement using data and client feedback
- Represent Community Forward in community and citywide meetings
Requirements
What you bring
- Master's degree in Social Work, Human Services, Public Health, or a related field
- Clinical licensure (LCSW, LMFT, Licensed Psychologist, or equivalent)
- Proven leadership experience managing social service or homelessness programs with complex teams
- Strong skills in strategic planning, budgeting, and compliance within nonprofit or government settings
- Deep understanding of homelessness, trauma, mental health, substance use, and systems of oppression
- Passion for equity, empowerment, and trauma-informed care
- Excellent communication, collaboration, and team-building abilities
- Proficiency with Office and G-Suite applications
- CPR and First Aid certification within 90 days of hire
- TB clearance and proof of COVID-19 vaccination within 7 days of hire
- 5150/5585 Certification within 180 days of hire
Preferred Qualifications
- Multicultural and bilingual, especially Spanish-speaking
- Experience working with gender-specific, at-risk populations
- Familiarity with San Francisco's billing systems (Epic), CalAIM, Medi-Cal
- Knowledge of local community resources and systems
