A Safe Place to Heal

A partnership supporting recovery and stability.


For someone experiencing homelessness, recovering from an illness, injury, or medical procedure can be extraordinarily difficult. 

Community Forward’s 75-bed Medical Respite bridges that gap by providing a safe space to heal, alongside medical care, case management, housing support, benefits assistance, and other resources. For more than a decade, Community Forward SF and the San Francisco Department of Public Health (SF DPH) have partnered to support unhoused people as they recover after hospitalization, combining SF DPH’s clinical expertise and healthcare resources with Community Forward’s experience in trauma-informed, low-barrier, community-based care. Last year, 394 individuals accessed support through the program–a reflection of what’s possible when two organizations bring their strengths together around a shared commitment to health, stability, and recovery.

That shared commitment recently reached an important milestone: our Medical Respite Center earned certification from the National Institute for Medical Respite Care (NIMRC), being the only organization to receive a perfect score. The certification, which was made possible with the support of a grant from Kaiser Permanente, recognizes programs that demonstrate a high degree of fidelity to nationally recognized principles and standards for medical respite care. Brandon Dawkins, Operations Manager for SF DPH, reflected on this milestone and what it means for his team and partnership with Community Forward.  

“This recognition instills a deep sense of pride, reinforcing that the staff’s everyday actions make a direct, meaningful impact on health equity in San Francisco,” shared Brandon. “It energizes our shared team to continue pioneering collaborative care models between public health and community-based organizations, inspiring us to continually elevate the standard of care for our unhoused neighbors.”

For our teams, it is both a recognition of the quality of care being provided and a reflection of the collaboration that has made this work possible.

That commitment to quality is reflected every day in the work of staff like Emerson Flores, Community Forward SF’s Director of Support Services. Before joining Community Forward, Emerson spent 22 years as a registered nurse in emergency medicine, giving him firsthand insight into what people often need after leaving the hospital.

“Medical Respite is work that can sometimes go unseen, but the impact we have on our clients’ lives is meaningful every day,” Emerson says. “We provide the essentials—meals, showers, medications, transportation to medical appointments, and so much more—so our clients can focus on their recovery after leaving the hospital.”

For the staff who make this work possible, the NIMRC certification is a meaningful affirmation of that daily dedication. It recognizes the nurses, case managers, frontline staff, and operations teams who work together to provide compassionate, high-quality care—and it reinforces the motivation to keep innovating and continue to pioneer collaborative care models between public health and community-based organizations.

At its heart, medical respite is built on a simple idea: healing is more possible when people have a safe place to do it. But creating that place takes partnership, and a shared commitment to elevating the standard of care for people experiencing homelessness.
